Traffic lights for Frederick & John Street intersection Ashfield Petition. By 29 March the petition was at 1521 signatures.  https://www.change.org/p/traffic-lights-for-frederick-john-street-intersection-ashfield

A number of minor improvements to Frederick St have been included in the Inner West Council PAMP adopted in April 2021 (however Council's role is limited on a state road) https://yoursay.innerwest.nsw.gov.au/pedestriansafety

Council also has previous resolutions relating to Frederick Street and residents at the community meeting also raised concerns with the crossing at Henry Street and the traffic lights at Church and Elizabeth St. The Tunnel under the main rail line is also the source of concern. I hear there was another accident on Frederick St today.

The current crossing was only 'upgraded' after Mehreen Faruqi asked a question in Parliament. June 2018 https://www.parliament.nsw.gov.au/hp/housepaper/16569/QA_160_06_JUNE_2018.pdf

Note the central median has not been implemented. From Report of Transport for NSW entitled "NSW Road Safety Progress Report 2018", dated June 2019 https://www.parliament.nsw.gov.au/tp/files/76502/NSW Road Safety Progress Report 2018.pdf
Frederick Street, Ashfield

This included the installation of kerb blisters, centralmedian, new line marking and signage to increase thevisibility of the crossing and to significantly improvepedestrian safety

"Following extensive inquiries, on Saturday 26 March 2022, the driver was issued a court attendance notice and charged with dangerous driving occasioning death – drive manner dangerous, negligent driving (occasioning death), and not give way to pedestrian on crossing – motor vehicle." https://www.police.nsw.gov.au/news/article?id=100748, viewed 11 April 2022
